Could this be the world's worst accident?

We've all had workplace accidents before. All of us.

We've broken coffee mugs, dropped the paperwork in the sink, maybe even blown up a curry in the work microwave. But what about this one!

It takes the cake.

We can obviously see what's happened but can you tell me what went wrong but only using "wrong answers"
